This Change of Condition Privacy Policy explains how Y Point Analytics (in partnership with Minuteman Senior Services) collects, uses, discloses, and otherwise processes personal information in connection with our Platform or any other interactions you have with this application as a prospective or active drivers/volunteers.  Learn more about our partnership at www.cocmeals.com

 

This Privacy Policy does not cover or address our meal recipients, employees, or other personnel. If you see any undefined terms in this Privacy Policy, you will find them specified in our Terms of Use. Further, this privacy policy is incorporated as part of the Terms of Use. 

 

By accessing and/or using the Platform or otherwise providing us with your personal information, you agree that you have read, understood, and agree to all the terms and conditions of this Privacy Policy, and specifically that you consent to the collection, storage, use and disclosure of your personal information as described in this Privacy Policy. If you do not agree to all the terms and conditions of this Privacy Policy, do not use the Platform, or otherwise provide us with your personal information. 


Personal information we collect:


User information

 

A user must be authorized to use our mobile application, prior to accessing it. The mobile application uses the username and password that was previously assigned to you by your organization to authenticate the user for access. Additionally, our app collects information from you based on your responses provided within the application. The data collected is used for providing senior care intervention by our partners Minuteman Senior Services.

Information about your device and its software


We
do not collect information about your device and its software which includes your IP address, browser type, Internet service provider, device type/model/manufacturer, operating system, date and time stamp, and any unique ID to identify your browser, mobile device, or your account or any other similar information. We do not collect mobile sensor data (such as bearing or direction, speed, latitude and longitude, altitude/height, activity type, and acceleration or deceleration) for location and analysis.


Location Information 


While your location information is not part of the data required to operate the app, each time you access or use the CoC mobile App, we may collect the location of your device when the Driver App is running in the foreground or background.


How we protect your data

 

We use reasonable organizational, technical, and administrative measures designed to protect against unauthorized access, misuse, loss, disclosure, alteration, and destruction of personal information we maintain. Unfortunately, data transmission over the Internet cannot be guaranteed as completely secure. Therefore, while we strive to protect your personal information, we cannot guarantee the security of personal information. In the event we are required to notify you about a situation involving your data, we may do so by email or telephone to the extent permitted by law.

 

Data Collection & Sharing Disclosure

 

  • This application does not collect any data about the user location.
  • This application does not collect any data about the user device.
  • Only information that we are collecting version number of the application, this is to ensure that we are not using the old version of the application.
  • Any consumer related change in health conditions that the user reports on this application is collected and shared with the care managers of the consumers. So that appropriate followup action items can be taken.
  • This application passes parking latitude and longitude to here maps to allow us to display all the locations in your delivery plan.
  • Any consumer change in health condition data that is provided by you, shared only with the Care Manager, Site Coordinator, Care Manager Supervisor, Nutrition Manager,Operational Coordinator. This data is protected by US HIPPA laws never shared with any individual or organization other than the Area Service Aging Provider(ASAP) that you are working for.
  • All the data that is collected by this application is transferred to our servers using HTTPS protocols.

 

Children

 

CoC (Change of Condition) is not directed towards individuals under the age of 13, and we do not intentionally gather personal information about visitors who are under the age of 13. Furthermore, we do not intentionally allow visitors under the age of 18 to register for the app. If a child under 13 submits personal information to us and we learn that the personal information is the information of a child under 13, we will attempt to delete the information as soon as possible. What choices do you have regarding your data?

 

You can make the following choices regarding your personal information:

  • Application. You can stop all collection of information by the app by uninstalling it. You may use the standard uninstall processes as may be available as part of your mobile device or via the mobile application marketplace or network.
  • Access or correct your information. You may request to review and update the information we maintain about you by submitting a request to info@ypointanalytics.com
  • Opting Out of location tracking. If you initially consented to the collection of geo-location information, you could subsequently stop the collection of this information at any time by changing the preferences on your mobile device. Please note, however, that if you withdraw consent to our collection of location information, you may no longer be able to use some features of the app.

This Privacy Policy is subject to occasional revision. We will notify you of any material changes in its collection, use, or disclosure of your personal information by posting a notice on the cocmeals.com website and within the CoC (Change of Condition) application.

Comments

When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.

An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.

Media

If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

How long we retain your data

If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.
