How was the app created?

The Change of Condition Application was built by Y Point Analytics in partnership with Minuteman Senior Services, based on feedback from Minuteman leadership, volunteers and caregivers.

This app is currently being used by around 700 volunteers who have delivered over 200,000 meals to 1,400 seniors in isolated neighborhoods.

    Change of Condition Project has been selected in the Nutrition category.

    • The n4a Aging Innovations & Achievement Awards
      Ceremony on Tuesday, September 22, 10:00-10:30 am ET. As many
      representatives as you’d like from your winning program can attend the
      virtual ceremony, even if they are not registered for the Virtual
      Conference, so feel free to invite the whole team.

    • General Session recognition: All 2020 Aging Innovations &
      Achievement awardees will receive recognition at the September 22
      General Session, when representatives from the program supporter,
      Centene, will announce the top-ranked Aging Innovations Award
      recipients, which will receive monetary awards. Only registered Virtual
      Conference attendees are able to attend the General Session.

    • Media Release: After the September 22 General Session, n4a’s
      press release on the program will spotlight the top Innovations winners
      and link to the full list of winning programs. To generate local coverage
      of your winning program, we also provide a sample press release for you
      to use locally.
